POLICIES & PROCEDURES

Medical Provider to Laboratory

• Ultrex will pay shipping and handling charges from provider to laboratory using the freight bills provided.
• Please include Rx form with each pair of casts that have been marked with the patients name. (Rx forms will be provided free of charge)
• Ship casts that are completely dry in a box of equivalent size with minimal packing material.
  • (Casts that are shipped wet or even damp will flatten out during shipment)                                                                                                         
  • (Ultrex will not be responsible for casts damaged in shipment)
• When ordering specialty orthotics or when ordering orthotics for for specialty footwear please send footwear whenever possible.

Laboratory to Medical Provider

• The standard turn-around time is 5 - 7 business days once Ultrex receives casts with a completed Rx form.
  • (The turn-around time can be increased significantly when the Rx form is received with incomplete information)
• 2nd pairs and phone orders: 3 - 5 business days. (Note: Rush orders will be assessed a $20 fee per order)
• A shipping and handling fee will be assessed as follows:
  • 1 pair           $20
  • 1 - 5 pairs    $20 for the entire shipment
  • 6 - 9 pairs    $25 for the entire shipment
  • 10 + pairs    shipping is free
  • Orthotics under warranty are shipped free
  • Overnight shipments add $10 per shipment

Casts and Patient Information

All casts will remain the property of Ultrex Laboratory and will remain stored for two months time before being destroyed. Digitized images of casts and all computer data will be kept for a minimum of 5 years. Hard copies of Rx forms and other patient related information will be digitally scanned and also kept for a minimum of 5 years before being destroyed.

Professional Courtesy

• Bronze Package
  • One pair, any style, of orthotics for the practitioner's use per year at no charge.
• Gold Package
  • One pair, any style, for the practitioner's use per year at no charge.
  • One pair, any style, for the practitioner's immediate family (spouse, 3 children limit) 50% off for the first pair, 30% off second pair, 3rd pair free.                  (Practitioner will be charged the difference for 2nd and 3rd pairs when specialty orthotics are ordered)
• Platinum Package
  • 5 free pair per month for the personal use of practitioners and their immediate family and/or staff.
